Uniparental disomy (UPD) occurs when both homologs of a chromosome originate from one parent. UPD of chromosome 20 (UPD(20)mat) is rare but associated with growth restriction, Silver-Russell-like features, and transient neonatal diabetes. Here we describe a couple, Marie (32F) and her partner Paul (34M), presenting with 4 failed IVF cycles. Paul exhibited extreme teratozoospermia – termed clinically “sperm mania” – characterized by >95% acrossmaless round-headed sperm (globozoospermia), hyperkinetic but non-fertilizing motility, and disordered chromatin. Marie was later found to have mosaic UPD(20)mat. We discuss whether maternal UPD can influence epigenetic programming of sperm quality via seminal plasma factors or shared genetic background.
